Belgrade, Montana — July 20, 2026Lindsey Klebenow, Gerontologist at Heart to Heart Home Care & Adult Day Center, has been named a winner of the 2026 Home-Based Care Momentum Awards in the category of Executive Leadership, presented by AlayaCare.

The Home-Based Care Momentum Awards recognize outstanding individuals across the home-based care industry who are driving meaningful change, elevating the standard of care, and making a lasting impact on the communities they serve. This year’s program drew nominations from across North America, with thousands of public votes cast and an independent judging panel selecting 60 winners across four categories.

Lindsey Klebenow was recognized for Executive Leadership through her honesty, integrity, kindness and perseverance.

“This honor belongs to every person who has trusted me with their story from my grandmother living with Alzheimer’s, to my husband navigating a traumatic brain injury, to the older adults, caregivers, and families I have had the privilege of serving throughout my career. They are the reasons I continue to advocate, innovate, and work to create meaningful change. Compassion is not simply caring for people when they need us, it is building systems that ensure they never have to face life’s greatest challenges alone. That mission has guided every step of my journey, and it will continue to drive my work as we strive to ensure every person has the opportunity to age with dignity, purpose, and hope.” – Lindsey Klebenow

“The people who deliver home-based care are among the most dedicated professionals in healthcare, and they deserve to be celebrated. What struck me about this year’s nominations wasn’t just the scale of the program, but the depth of the stories behind each person. These 60 winners represent something important: that excellence in home-based care is happening everywhere, in every role, at every level of an organization. We are proud to shine a light on them.”
